- Intake + goals i We’ll review the situation, identify top priorities, and define clear goals for the next 1–2 weeks.
- Priority list of next steps i A short, actionable plan: who to call, what to request, what to track, and what decisions are coming.
- Appointment prep checklist i A checklist for the upcoming visit: questions, documents, symptoms to track, and what to bring.
- 1 follow-up message (email) i One follow-up email to answer quick questions and confirm next steps after you’ve started the plan.

Care Transition Package
- Intake + discharge review i We review discharge instructions, new diagnoses, meds, follow-ups, and identify gaps before they cause problems.
- Medication & follow-up checklist i A checklist to track medications, refills, follow-up appointments, and tests—so nothing gets missed.
- Questions for providers + red flags to watch i We outline what to ask at follow-ups and what symptoms/changes should prompt urgent medical attention.

- Appointment prep + next-step coordination i Visit preparation plus coordination of referrals, labs, imaging, follow-ups, and questions to ask.

Are you a medical provider?
No. ClearPath provides non-clinical care navigation, coordination, and education. We do not diagnose, treat, prescribe, or provide emergency services.
Do you talk to doctors for me?
We can support communication and preparation. With proper written authorization, we may help organize questions and facilitate care coordination, but medical decisions remain with the treating providers.
Is this a substitute for home health or caregiving?
No. We do not provide hands-on nursing care, home health services, or emergency response. We help families navigate, organize, and follow through.
What if this is urgent?
If this is a medical emergency, call 911. ClearPath response time is within one business day for non-urgent needs.
